Much I really liked in the post, Todd -- especially establishing a Profile of a Learner as the initial step. I have to admit that I try to always resist the temptation to use the term personalized learning when what is really being offered is personalized instruction based on a preset curriculum. That means I also consider improved scores on standardized tests to be evidence of that personalized instruction not learning. Standardized tests can't measure learning -- they can only measure the effectiveness of instruction.
